Bubble Sheet

Generic Bubble Sheet / Answer sheet Response Templates

These are examples of some OMR bubble -answer sheet templates designed using FormReturn. An ID grid is added and used to identify the respondents by marking in their ID number. Each template available for download can be customized by changing aggregation rules and adding text to the heading, and used to automatically process returned response data using FormReturn OMR software installed on your computer.

The zip file contains the form template segment file(.frs) and form file(.frf). These are single page forms.

There are 2 paper sizes to choose from, A4 and Letter.
An OMR ID grid is used for collecting respondent's ID numbers.
