Advantages of using Assessment Software for Multiple-choice Examinations, Tests and Surveys
- Multiple-choice questionnaires can cover a wide variety of subjects in a small amount of time, so are a popular way of testing student's capabilities. Examiners can create customized multiple-choice questionnaire forms quickly and easily using FormReturn's form editor.
- Minimize cheating by creating unique, customized forms for students, using banks of questions randomly placed on the form.
- When a student gets the easy questions correct but misses the difficult ones or another student misses easy questions but gets difficult ones correct, composite test scores can be misleading when measuring ability. A feature of FormReturn is mark aggregation. A higher mark for the correct answer can be assigned to the corresponding box as well as penalizing with negative marks if the incorrect box is marked. For examinees who were guessing, this would make it difficult to still get a high score.
- Check to see all forms have been returned. When forms created and published on FormReturn are scanned back into the system, you can see which ones haven't been processed. FormReturn can customize each form by using student's records in the FormReturn database or use an ID grid for matching with data records using OMR.


Each page of a FormReturn form template must contain 2 things:
- A Form ID Barcode to identify the form template and the publication the form belongs to.
- One or more segments containing the checkbox/bubble mark areas, for gathering response data which will be processed by the OMR reader.
Optional features for identification of forms are either Template Variable Replacement (prints records from your data table on the forms) or an OMR ID grid (for marking in a student ID, added to each page of the form template) or a Barcode Area (used to affix a barcode label of your own choosing).
Example of how FormReturn can be used to design and publish basic form templates.
- A Basic customized multiple-choice assessment form template can be designed using any layout you want, with as many checkboxes as you need, on one or more pages. Even import text from existing forms onto a FormReturn form template.
- Add graphic designs using the editor's tools (great for maths) and insert images to create a truly customised form design.
- Segments containing the questions and mark areas are designed separately and added to the form template. One or more segments can be added in which ever order you choose, or choose to have them randomly placed throughout the publication (eg: every form will contain the same banks of questions but they will be ordered differently on each form).
- Barcodes are easily added to each form template page and used for identifying the form template and linking the captured data to the student records in the data table.
- The form template is published, assigning it to a data table containing the student's records.
- Print unique forms for distribution to each student, or use another feature of FormReturn (OMR ID) which allows you to pre-print or photocopy the published form template for random distribution. This feature uses a student ID number marked into a grid, to link returned forms back to the student's record in the data table.
Example of how captured data is reconciled: In a class situation, each student will have a record in the data table, containing their first name, last name, student ID and so on. When an examination form template is published for the class, the data table containing the records for the class is selected to link the publication and it's captured data to. Each examination form page that is published, has a Form ID Barcode that contains the form template information and the data record information. When the returned forms are processed, each captured form page of data will link back to the record it belongs to in the data table. When you click on the page in Captured Data, you will be able to see the processed response data from that page.
About the Multiple_Choice Form Templates available for download
- These are some examples of the types of OMR forms that can be designed in FormReturn and used to grade assessments automatically.
- The PDF File is for viewing the template design only. Any changes to the template can be made using the segment and form template editors.
- Each template zip file contains the segment file/s and the form template file. Any changes to the text or aggregation rule in the segment file means it will have to be saved and then re-added to the form template. (To do this you will have to delete the segment from the Form template file, SAVE, and then re-add the segment files)
- Download any of the templates, save on your computer, and open in
FormReturn's template editors. Replace text and add a mark aggregation rule. Use the Help Guide and follow the basic steps for form design, recognition testing and publication of the template.
