Each of these form templates has a choice of either A4 or Letter size paper. Template Variable Replacement, a feature of FormReturn for printing unique personalized forms using the records from your data table, has been included in the design. The automatically captured response data from each form will be reconciled with it's record in the data table.
Each template available for download from the table, is a Zip file containing the form file and segment files.
