Introduction:

FormReturn Optical Mark Recognition software detects hand marked responses, made in checkboxes on questionnaire forms, then transforms them into readable results which can be transferred to a printable spreadsheet and it has the ability to process thousands of bubble sheet response forms a day.
Product History:
FormReturn is a newly released (Sept 2009) Optical Mark Recognition software product, which has been in creation since 2006. It is the result of extensive research into the development of a simplified approach to automatically collecting results from questionnaire response forms.
The need for a less expensive and easier to use Optical Mark Recognition solution, for small, medium and large organizations was evident, which prompted the creation of an uncomplicated to use yet effective software solution. FormReturn allows industries and institutions which rely on the collection of data from multiple-choice response forms, to operate more efficiently, alleviating the need for spending time analyzing responses from each individual form and spending more time interpreting the results and assembling them into reports and available information lists, which is the most important aspect of the data collection.
Optical Mark Recognition Form Processing:
Form Return's form processor recognizes forms that have been created using the software's template editors, to add barcodes to the form template which are used for form identification and the location of checkbox mark areas.
When questionnaire forms are returned from respondents, they are scanned and uploaded into FormReturn for automatic analyzing and capturing of hand marks in checkboxes into readable data, which can then be used to collate your reports.
FormReturn uses barcode recognition to link the captured results (which checkbox was marked and the given score) with the records in your data table and the form template, which was published using those records.
The printable spreadsheet produced, will contain the respondent's records from your chosen data table and columns showing the question name, which checkbox was detected as marked in each question and the score given, if one was allocated.
Special Features:
The FormReturn software has many powerful, useful features such as Template Variable Replacement which can print unique forms for recipients, using your chosen data table records.
Mark Aggregation can be set up to allocate a score when the correct checkbox is marked. Mark Aggregation can also be used to allocate a negative score when the incorrect checkbox is marked which can help prevent a higher score for students who were only guessing.
Respondent ID number recognition can also be used, so that anonymous forms can be pre-printed or photocopied and distributed on demand. It works by creating a grid for filling out by the respondent with an ID number. The grid is set up similar to a checkbox mark area. The ID number is recognized by the form processor as a string and then matches it with the corresponding record in the data table.
All Optical Mark Recognition forms need to be recognized by by their own form processor. FormReturn lets you recognition test your scanned form images and alter the standard recognition settings if needed, so your returned form images will be detected accurately.
All of these exceptional features of FormReturn are very easy to understand and use with some practice. The online help guide is excellent for step by step instructions for using the software and there is also technical support on demand if you are still having difficulty using the software.
Template Editor for Form Design:
Of all the Optical Mark Recognition software on the market, FormReturn has the most flexibility when it comes to form design.
Easily design your own customized checkbox questionnaire forms using the template editors to quickly add everything that you need to the form template, to design the type of response form you are used to using.
Because it has been proven that an oval bubble is easier for respondents to quickly and accurately fill in, the form editor lets you quickly add standard size, oval checkboxes to the template wherever you want.
Text areas are easily added for typing.
Use the range of alignment tools or the duplicate feature so your form layout is neat and quickly finished.
