OMR Template Editor
Create your own customized OMR forms in FormReturn, using the segment and form template editors. FormReturn has built-in template editors, (they are the Segment Editor and the Form Editor) which allows you to write, draw, import graphics, add barcodes and most importantly, easily add multiple checkbox bubbles which can be read by the software.
Basic Form Structure:
- Form ID Barcode: This barcode is used to let
FormReturn know the publishing information about this form (such as
which form template was used, which segments it contains, what is the
published form template it relates to etc). From there, FormReturn will be able
to determine the form structure for processing. You must always have
this barcode in every page processed by FormReturn.
- Segment Area: The area from the corner of the top right segment barcode to the corner of the bottom left segment barcode is the segment area. The Segment is designed separately and added to the form template. The purpose of using segment barcodes to define a physical area gives the image scalability.
- Fragment (Mark Area): A fragment is each checkbox mark area and the immediate area surrounding it on the segment. Usually many fragments will appear in a segment. Fragments are the area that FormReturn scans to detect which options have been marked by the respondent of the form.

Segment Editor
As discussed in the segments feature page, segments contain the checkbox mark areas used for the optical mark recognition capture of response data. Use the checkbox tool in the segment editor toolbar to quickly and easily add checkboxes to the segment form. Use the text tool to add text areas for questions and start typing.

Form Editor
The Form Editor toolbar contains the tools needed to design a customized form template.

Use the Segment tool in the Form Editor toolbar, to create an empty segment area on the form template for segments to be loaded into. Select the Segment tool then click and drag an area on the form where the segment is to be loaded. Click and drag to make the area larger and move it around on the form or use the Element panel at the left. To load segments see Part 2 of the Quick Start Guide.
Use the Barcode tool to add a Form ID Barcode. A Form ID Barcode must be added to every page of a form template for identification purposes. FormReturn uses the form ID barcode to reconcile the captured response data with the data records of the form's respondents and stores them together in Captured data.
