Bubble Sheet Templates

The bubble sheet templates, available to download from the table below, can be used with FormReturn OMR software. They are examples of the kinds of bubble sheets that can be designed using FormReturn's OMR Form Editors.

Download the template zip files, save on your computer, and open in FormReturn's template editors where they can be customised, for your own use. Each zip file contains a segment file.frs and a form file.frf. Use FormReturn's online Help Guide for full instructions on using the software. There are 2 form paper sizes to choose from, A4 and Letter.

There are 2 form identification types to choose from, Template Variable Replacement and OMR ID grid.

Bubble sheet / Answer sheet templates

Download a Pdf Guide for Templates with Template Variable Replacement used for form identification.

Download Pdf Guide for Templates with OMR ID Grids used for collection of ID numbers for form identification.

Templates 1 through to 4 are Education: multiple_choice bubble sheet questionnaire.
Templates 5 through to 7a are Education: multiple_choice response forms... Answer sheet / bubble sheets only.
Template 8 / 8a is a Conference Evaluation Survey / multiple_choice bubble sheet questionnaire.
Template 9 / 9a is a Health Care: Patient Survey / multiple_choice bubble sheet questionnaire.
Template 10 / 10a is a Market Research: General Customer Satisfaction / multiple_choice bubble sheet questionnaire.
